Primary care provider
The mission of Mobile Care Partners of Connecticut is to keep our community healthy by providing primary care where the people need it most. Our kind, caring staff work diligently with patients, family members, and community organizations to ensure the highest quality care is offered. We don't know our patients by their room number or diagnosis - we know them as the farmer with the big friendly dog, or the mother working hard to balance a healthy lifestyle and busy home/work schedule.
Siena Giordano graduated with her Doctor of Nursing Practice from Quinnipiac University.
She has been part of the healthcare community for 10 years, first working as a Certified Nursing Assistant then a Registered Nurse before becoming a Nurse Practitioner. She previously worked in a Primary Care office treating adults in Litchfield County. She has a special interest in preventative medicine, mental health and chronic disease management.
She is delighted to be part of Mobile Care Partners and have the opportunity to provide quality healthcare to members of the community. She enjoys getting to know each patient in the comfort of their home while creating a partnership to bring about positive change in people’s lives.
Siena has settled in the Northwest Corner where she was born and raised. Outside of work, she enjoys spending time with family including her son, cooking, spending time outside and decorating her home.
